RESUMO

La propuesta que se presenta es fruto, por un lado, de los resultados del proyecto “IPACAT22: Consenso sobre los aspectos clave del modelo de la práctica avanzada en Cataluña”, realizado con la participación de 24 enfermeras representantes de asociaciones científicas enfermeras o multidisciplinares de Cataluña. Por otro lado, es también fruto del apasionante, relevante y a veces difícil debate entre los miembros del equipo con diferentes trayectorias profesionales y distintos puntos de vista que enriquecen el objetivo común. Es oportuno presentar los resultados del debate, basados en el rigor, y los resultados de los proyectos de investigación por la mejora y el avance de la enfermera de práctica avanzada. Este debate también desempeña un rol importante en el entorno profesional para ejercer un liderazgo efectivo en las organizaciones y en el sistema de salud. Estamos convencidas de que este documento puede ser un punto de partida para configurar el modelo de la enfermera de práctica avanzada en nuestro contexto.(AU)


The proposal presented is the result, on one hand, of the outcomes of the Project “IPACAT22: Consensus on the key aspects of the model for advanced practice in Catalonia”, prepared with the participation of 24 nurses representing multidisciplinary or nursing scientific associations of Catalonia. On the other hand, it is also the result of the exciting, relevant, and sometimes difficult discussion among the team members with different professional careers and point of views which enhance the common objective. It is appropriate to present the outcomes of this discussion, based on rigour and results of the research projects for the improvement and progress of advanced practice nurses. This discussion also represents a major role in the professional setting, to conduct an effective leadership in organizations and the health system. We are convinced that this document can be a starting point to set up the model for the advanced practice nurse in our setting.(AU)

Objetivo: identificar, pelas evidências científicas na consulta do enfermeiro em cuidado avançado, o emprego das terminologias padronizadas nas etapas do processo de enfermagem em contextos de cuidado. Método: revisão integrativa realizada em seis etapas. Resultados: foram selecionados 14 artigos. As terminologias padronizadas mais utilizadas pelo enfermeiro durante as consultas de enfermagem foram a CIPE® (65%), seguida da NNN ­ NANDA-I, NOC e NIC (21%); NOC (7%); CIPE®, NNN e Omaha System (7%). 43% utilizam a linguagem padronizada nas etapas de diagnóstico, resultado e intervenção. A respeito do contexto de cuidado, o uso de terminologias padronizadas de enfermagem era empregado 36% em ambiente hospitalar, 14% em Home Care, 14% em Atenção Primária à Saúde, 7% em clínica privada e 29% não foram informados. Conclusão: a terminologia padronizada mais utilizada foi a CIPE® (65%), nas etapas de diagnóstico, resultado e intervenção (43%) no ambiente hospitalar (36%).


Objective: to identify, through scientific evidence in the consultation of nurses in advanced care, the use of standardized terminologies in the stages of the nursing process in care contexts. Methodology: integrative review carried out in six stages. Results: 14 articles were selected. The standardized terminologies most used by nurses during nursing consultations were ICNP® (65%), followed by NNN-NANDA-I, NOC and NIC (21%); NOC (7%); ICNP®, NNN and Omaha System (7%). 43% use standardized language in the stages of diagnosis, outcome and intervention. Regarding the care context, the use of standardized nursing terminologies was used by 36% in a hospital environment, 14% in Home Care, 14% in Primary Health Care, 7% in a private clinic and 29% were not informed. Conclusion: the most used standardized terminology was ICNP® (65%), in the stages of diagnosis, outcome and intervention (43%) in the hospital environment (36%).


Objetivo: identificar, por medio de las evidencias científicas en la consulta del enfermero en cuidado avanzado, el empleo de las terminologías estandarizadas en las etapas del proceso de enfermería en contextos de cuidado. Metodología: revisión integradora realizada en seis etapas. Resultados: se seleccionaron 14 artículos. Las terminologías estandarizadas más utilizadas por las enfermeras durante las consultas de enfermería fueron CIPE® (65%), seguida de NNN ­ NANDA-I, NOC y NIC (21%); NOC (7%); CIPE®, NNN y Omaha System (7%). RESUMO

Pediatric Hospital Medicine (PHM) is a growing subspecialty with a broad scope. The Covid-19 pandemic demands flexible staffing models. Advanced practice providers (APPs) can be a valuable addition to hospital medicine teams, although there is no established training program for APPs within PHM. The authors' purpose is to describe how one institution rapidly established a PHM APP team by collaborating with experienced APPs working in other areas of the hospital. This APP team cared for 16% of the average daily census during the pilot period with no significant difference in length of stay compared to traditional teams.

BACKGROUND: Midwifery practice is essential in achieving high-quality maternal and newborn care in all settings and countries. However, midwifery practice has become more complex over the past decades. Considerable demands are being placed on midwives to meet increasing epidemiological, socio-economic, and technological challenges. These require a well-trained midwifery workforce ready to shape the care in the near and long-term future. OBJECTIVE: To discuss advanced midwife practitioner role implementation in Belgium as a possible answer to healthcare-related challenges that impact midwifery practice. Furthermore, to stimulate a debate within the profession at all levels in Belgium and in countries considering advanced midwife practitioner roles. METHOD: The framework by De Geest et al. (2008) served as a basis for discussing the drivers for advanced midwife practitioner role implementation: the legal, policy and economic context, workforce issues, education, practice patterns, and healthcare needs of the population. FINDINGS: A legal basis for advanced midwife practitioner role implementation is lacking in Belgium. Remuneration opportunities for the non-clinical part of these roles (e.g. leadership and innovation activities) are missing. It might be challenging for healthcare organisations to support the implementation of such roles, as immediate revenues of non-clinical activities are absent. However, sufficient potential resources are available to fill in future advanced midwife practitioner positions. Additionally, advanced midwife practitioner specific master programmes are being planned in the near future. CONCLUSIONS: Although several barriers for the implementation of advanced midwife practitioner roles were identified, a discussion should be held on the opportunities of implementing these roles to facilitate the development of new models of care that meet current and future challenges in midwifery practice and healthcare. After initial discussions amongst midwives in academic, managerial, and policy positions, stakeholders such as obstetricians, general practitioners, associations representing healthcare organisations, and policy makers should be involved as a next step.

RESUMO

The US health care system has seen unprecedented growth in health care cost with only a mediocre return on investment. Achieving sustained quality improvement will require innovation that is effectively integrated into complex systems of care. Complexity leadership has the ability to place value on traditional quality improvement processes, with less focus on rigid structure and more attention on the potential for flexibility and creativity at the point of care. Clinical team leaders, such as nurse practitioners, must nurture adaptability to the constantly changing clinical environment while balancing structured thinking of team members. Nurse practitioners operating in interdisciplinary teams are well positioned to foster adaptive change through rapid cycle improvements at the point of care. As a learning approach to quality improvement, the PDSA (plan-do-study-act) method should be seen as a useful tool for organizations to create an emergent quality improvement process. This article discusses the significance of nurse practitioner leadership using the PDSA method informed by complexity leadership theory and the impact for clinical practice.

BACKGROUND: Advanced practice registered nurses (APRNs) play a pivotal role in meeting the increasing needs of elder care given the aging population. A good understanding of the characteristics of gerontological APRNs is important for future workforce planning. PURPOSE: To understand the demographic, employment, and practice characteristics of APRNs who provide elder care. METHODS: A 34-item survey was distributed to 2,500 current members of the Gerontological Advanced Practice Nurses Association. Three hundred and sixty-four members provided demographic information and employment and practice patterns. To examine the sample representativeness, the survey sample was compared with a sample drawn from the National Sample Survey of Nurse Practitioners 2012. Descriptive statistics were used for analysis. RESULTS: Most respondents were older than 55 years, held a master's degree, were certified as a Gerontology APRN, worked in a long-term/elder care setting as an APRN, earned a salary ranging from $105,001 to $125,000, and practiced in urban or suburban areas. Most respondents worked full time, provided care for people who were 55 to 75 years old, and prescribed under a collaborative agreement. IMPLICATIONS FOR PRACTICE: The study results suggested that gerontological APRNs have the experience and competence to meet the increasing needs of elder care, practice in a variety of settings, including in rural areas, and spend much time with patients. Future efforts are needed to support APRN practice, such as improving their autonomy and independence.

Advanced practice registered nurses (APRNs) are in an excellent position to provide leadership for advancing telehealth initiative. King's social system concepts of role, status, authority, power, and decision-making may be explored in this context. The authors of this article provide insight into the leadership opportunities in telehealth that exist for APRNs in current healthcare systems through a King perspective. The authors discuss some of the major challenges for APRNs in telehealth and how leadership may overcome some of those challenges.

The words used to describe who and what NPs are can affect efforts to achieve full practice authority throughout the United States. They affect how NPs treat each other and how the public, professional colleagues, and others view them, which can have a profound effect on rules, regulations, and policies that affect NP effectiveness and practice.

BACKGROUND AND PURPOSE: The Advanced Practice Registered Nurse (APRN) Compact was approved at the National Council of State Boards of Nursing Delegate Assembly in May 2015. Since that date, only three states have been successful in the legislative adoption of the compact. Thus, the purpose of this study was to explore the experiences and perceptions of nursing regulatory leaders involved with the individual state adoption of the APRN Compact. METHODS: This qualitative study followed a phenomenological-hermeneutic methodology approach. CONCLUSIONS: Four themes emerged from within the data: legislative political environment; professional organizational barriers; strategies to push past barriers; and moving forward. An overarching theme of consistent communication was evident. IMPLICATIONS FOR PRACTICE: This study identifies barriers experienced in the legislative state adoption process and successful strategies to overcome these barriers to move the APRN compact forward.

AIM: To describe the enablers and challenges to the development and implementation of advanced nursing and midwifery practice roles in Ireland. BACKGROUND: Leadership strategies need to be put in place to enhance the development and implementation of advanced nursing and midwifery practice roles. METHOD: A descriptive qualitative approach using semi-structured interviews with key stakeholders (n = 15) was undertaken with nurses and midwives working in specialist and advanced practice roles and participants from other areas such as legislative, regulatory, policy, pharmacy, medicine and education. RESULTS: Participant's perspectives on the enablers and challenges to enacting specialist and advanced practice roles resulted in the generation of three themes: organisational factors; collegial, interprofessional and interpersonal support; and role clarity, economic and regulatory contexts. CONCLUSION: Addressing organisational factors, encouraging collegial and interprofessional support and establishing role clarity contribute to the effective development and implementation of the role of advanced practitioners. IMPLICATIONS FOR NURSING AND MIDWIFERY MANAGEMENT: Managers of nursing services need to provide leadership in developing strategies to enhance the enablers and overcome the challenges to advanced practice role development in their own organisation.

Doctor of Nursing Practice (DNP) faculty play a critical role in preparing students to meet the complex needs of the nation as the number of cancer rates and survivors rise (National Cancer Institute, 2018) and as an unprecedented number of older Americans enter into the healthcare system with complicated comorbidities (Whitehead, 2016). Palliative care has dramatically expanded over the past decade and has been increasingly accepted as a standard of care for people with cancer and other serious, chronic, or life-limiting illnesses. Advanced practice registered nurses (APRNs) are recognized as important providers of palliative care (Walling et al., 2017). A 2-day course was held with support from the National Cancer Institute to enhance integration of palliative oncology care into DNP curriculum. The course participants (N = 183), consisting of DNP faculty or deans, practicing DNP clinicians, and students, received detailed annotated slides, case studies, and suggested activities to increase student engagement with the learning process. Course content was developed and delivered by palliative care experts and DNP faculty skilled in curriculum design. Participants were required to develop goals on how to enhance their school's DNP curriculum with the course content. They provided updates regarding their progress at integrating the content into their school's curriculum at 6, 12, and 18 months post course. Results demonstrated an increase in incorporating oncology palliative care in DNP scholarly projects and clinical opportunities. Challenges to inclusion of this content in DNP curricula included lack of: perceived time in curriculum; faculty educated in palliative care; and available clinical sites.
